Here is the player registration form.  Besides the usual contact information, our league has two seasons to offer and some other buy-in options for the season.  The nice thing about tracking the registration information is having the ability to use it’s data for future queries of the system.  I’ve been able to resolve a number of questions in the planning stages for our league.  For example, How effective is our on-line registration option?  How many registrations did we receive after this point last year?  How many players opted for one fund-raiser type over another?

Also shown on the bottom of the form is all of the players prior rankings.  Divisional commissioner have read-only access and are able to research players as they are putting the teams together.  Seeing all of the ranking data, helps you determine if a player is really a strong player or is more hit or miss.  You can also see below we ask all team coaches to rank the player.  In that way, we as a league get a better idea of the caliber of player.  Like anything else, some coaches are more critical than others.  Our ranking system runs 1-5, with 1 being a novice, 3 being average, and 5 being a “super-star.”
